Radiology Quiz August 2025

In collaboration with Dr. Adrien-Maxence Hespel from the University of Tennessee, we present the following radiology quiz. The goal of this case is to practice identifying radiographic findings and formulating a radiographic diagnosis based on a brief patient description. The final section provides additional information about the treatment.

Description of the RX images

Orthogonalβ€―radiographs of theβ€―thoraxβ€―are available for interpretation.

Within theβ€―cranialβ€―mediastinum, there is anβ€―ovoid, homogeneouslyβ€―soft tissueβ€―opaqueβ€―structureβ€―which measures approximately 5.0 cm inβ€―lengthβ€―and extends from theβ€―secondβ€―intercostal spaceβ€―to theβ€―fifthβ€―intercostal spaceβ€―along the left aspect of the cardiac silhouette. The cardiac silhouette andβ€―pulmonary vesselsβ€―areβ€―normal.

The pulmonaryβ€―parenchymaβ€―isβ€―normal.

Theβ€―pleural spaceβ€―isβ€―normal.

There is mild,β€―periarticularβ€―new bone formation at both glenohumeral joints. There is mild, multi-focal, incidentalβ€―spondylosis deformans.

Within the included portion of theβ€―abdomen, theβ€―liverβ€―is mildlyβ€―enlargedβ€―extending beyond the costal arch.

Theβ€―ovoidβ€―soft tissueβ€―opaqueβ€―structureβ€―within theβ€―cranialβ€―mediastinumβ€―may represent aβ€―malignantβ€―processβ€―(such as aβ€―thymoma) or a branchial cyst. The planned thoracicβ€―ultrasoundβ€―may provide further information.

– Mildβ€―hepatomegalyβ€―is non-specific, but does raise concern for hepatic lipidosis given theβ€―reportedβ€―hyporexia and anorexia.β€―Malignancyβ€―(such asβ€―infiltrativeβ€―neoplasia) cannot be excluded.

– Mild,β€―bilateralβ€―glenohumeral degenerativeβ€―jointβ€―disease.

On the annotated image the cardiacβ€―marginβ€―remains visible when superimposed with theβ€―caudalβ€―structure. This indicates that those two are not of the sameβ€―opacity. Furthermore theβ€―caudalβ€―structureβ€―is slightly moreβ€―radiolucentβ€―compared to the cardiac outline.
