Radiology Quiz November 2025

In collaboration with Dr. Adrien-Maxence Hespel from the University of Tennessee, we present the following radiology quiz. The goal of this case is to practice identifying radiographic findings and formulating a radiographic diagnosis based on a brief patient description. The final section provides additional information about the treatment.

Description of the RX images

Study: Leftβ€―lateral,β€―rightβ€―lateral, and ventrodorsal abdominal radiographs are available forβ€―review.

Findings: Theβ€―stomachβ€―isβ€―distendedβ€―with aβ€―largeβ€―volumeβ€―ofβ€―fluid, aβ€―smallerβ€―volumeβ€―of gas, and aβ€―smallβ€―number of pinpoint, mineral opaque foci. In the oradβ€―duodenum, seenβ€―ventralβ€―to the last pair of ribs in theβ€―lateralβ€―views and to theβ€―rightβ€―of L2 in the ventrodorsal view, there is anβ€―ovoidβ€―structureβ€―in the duodenalβ€―lumen. Theβ€―structureβ€―is gas-filled with aβ€―soft tissueβ€―opaqueβ€―borderβ€―surrounded by aβ€―thin rimβ€―of gas. It measures approximately 2 cm inβ€―lengthβ€―and 1 cm inβ€―diameter. Theβ€―duodenumβ€―orad to thisβ€―structureβ€―is mildlyβ€―distendedβ€―while theβ€―duodenumβ€―aborad to thisβ€―structureβ€―appears empty/smallβ€―inβ€―diameter. The remainder of theβ€―smallβ€―intestinal tract is mildly, uniformlyβ€―distendedβ€―withβ€―fluidβ€―and gas in a string of pearls pattern, consistent withβ€―peristalsis. Theβ€―colonβ€―contains aβ€―smallβ€―volumeβ€―of nonformed stool. The serosal detail,β€―liver,β€―spleen, kidneys, retroperitoneal space, andβ€―urinary bladderβ€―areβ€―normal. The imaged skeletal structures andβ€―caudalβ€―thoraxβ€―areβ€―unremarkable.

Conclusions:β€―Ovoidβ€―structureβ€―(suspectedβ€―gas-filledβ€―foreign body) in the oradβ€―duodenumβ€―causing intestinalβ€―obstruction. Surgicalβ€―interventionβ€―is warranted.
